Successions, Inc., established in
2004, was dedicated to providing quality Case Management Services to
adults with severe and persistent mental illness. In March of
2006 we moved from providing Case Management Services to providing
Community Support Services and Diagnostic Assessments to this same
population.
Successions, Inc. is contracted with Mecklenburg Area Mental Health,
Developmental Disabilities and Substance Abuse Authority to provide
community support services and diagnostic assessments for consumers in
Mecklenburg County.

Successions, Inc.’s
community support services program assists consumers in attaining
their goals to become as independent, functional, and productive as
possible. We use a person-centered approach to goal
setting and achievement. This means that every consumer has an active
voice in their treatment since it is the consumer who best knows what
they want to achieve and what motivates them to accomplish their
goals.
Successions, Inc. assists consumers by seeing that needed services,
support, skills training, community education and resources are
available to them. Our Community Support
Professionals serve as the “clinical home” to each consumer to ensure
that they develop the resources they need while providing a stable
support system of care.

needs to live full and satisfying lives as independent and responsible
citizens in their desired community.

toward and successfully achieving their life goals as they learn to
effectively and responsibly manage their mental health and substance
abuse treatment needs. We strive to accomplish this by providing
quality treatment services using a person-centered approach.

Successions has developed teams with more than
one Community Support Professional to care for specific populations.
A Qualified Professional who ensures that we continue to give
excellent care to each consumer leads each team. Our teams receive
specialized training and support to treat such populations as battered
women, substance abusers, consumers with housing issues as well as a
host of other specialized needs.
Our desire is to continue to become more and more
culturally diversified to reach populations that may not currently
feel comfortable accessing needed services. We plan to grow one
consumer at a time, following our person-centered approach and
striving for excellence and success. It is our sincere hope that our
efforts lead to the betterment of our consumers and our community as a
whole. |

Successions,
Inc. is located at 11145 Metromont Parkway in Charlotte, off Harris
Blvd. and Statesville Road. Our Community Support Professionals are
based out of this office. Our facility is a modern and comfortable
facility that has all ADA accommodations.
For an individual to be eligible to receive
community support services from Successions, Inc., they must first be
enrolled as a consumer of the Mecklenburg Area Mental Health,
Developmental Disabilities and Substance Abuse Authority (AMH). To
